Reference: Barkworth, Campbell, & Salomon in FNA (2007a); Campbell (2000); Church (1967); Tucker (1996)=Z; Barkworth (1997)=X. This treatment largely follows Barkworth, Campbell, & Salomon in FNA (2007a).
